BREAST AUGMENTATION INTRODUCTION

Breast augmentation is a cosmetic procedure that uses implants to enlarge and shape a woman’s breasts. For many women, breast size is an important part of feeling normal, self-confident, and sexually desirable. PATIENT 1 - Breast Augmentation

22 yo woman, 5’5"tall, weight 105 pounds. Pre-operative breast size is smaller than a 32A-cup. Six month post-operative photographs showing results of a bilateral dual plane-one, sub-pectoral breast augmentation with McGhan style 68 smooth, round, moderate profile (210cc) implants, placed via an inframammary fold incision, and filled to 250cc. Post-operative bra size is a full 32C-cup.

PATIENT 2 - Breast Augmentation

This 40 yo woman, 5’8” tall, weight 145 pounds, wore a 36C-cup bra prior to having children and liked her breast fullness; following her pregnancies she wore an A-cup. Examination showed a loss of breast volume with a lax skin envelope, but no true ptosis. Six month post-operative photographs showing results of a bilateral sub-pectoral, dual plane-one breast augmentation with McGhan style 68 smooth, round, moderate profile (300cc) implants, placed via an inframammary fold incision, and filled to 350cc. A bilateral circumareolar mastopexy was also performed for breast ptosis. Post-operative bra size is 36C-cup.

PATIENT 3 - Breast Augmentation

23 yo woman, 5’4” tall, weight 135 pounds. Her pre-operative bra size was a small 34B-cup and she desired a full C-cup. Two year post-operative photographs showing result of a bilateral, dual plane-one sub-pectoral breast augmentation with McGhan style 68 smooth, round, moderate profile (240cc) implants, placed via an inframammary fold incision, and filled to 270cc. Post-operative bra size is a full 34C-cup and she remains extremely happy.

PATIENT 4 - Breast Augmentation

21 yo woman, height: 5’0”, weight: 100 pounds. Pre-operative bra size is a 32B-cup. Seven week post-operative photographs show the result following a bilateral sub-pectoral breast augmentation with Mentor style 1600 smooth, round, moderate profile (325cc) implants, placed via an inframammary fold incision, and filled to 345cc on the right and 335cc on the left. Post-operative bra size is a full 32C-cup.

PATIENT 5 - Breast Augmentation

This 30yo woman, 5’6” tall, weight 130 pounds, wore a 36B-cup bra. During her pregnancies her breast size increased to a C-cup, but she wanted augmentation to a full D-cup without an excessively round breast shape. Her examination revealed some laxity of the breast skin envelope without true ptosis. Six week post-operative photographs show the results of a bilateral dual plane-one, sub-pectoral breast augmentation with McGhan style 468 textured, contoured (380cc) implants, placed via an inframammary fold incision and filled to 410cc. She loves her 36D-cup size.

PATIENT 6 - Breast Augmentation

37 yo woman, 5’3” tall, 130 pounds. Pre-operative bra size was 34A-cup. Six month post-operative photographs showing the result following a bilateral sub-pectoral breast augmentation with Mentor style 3000 smooth, round, high profile (380cc) implants, placed via an inframammary fold incision, and filled to 400cc. Post-operative bra size is 34C-cup.

PATIENT 7 - Breast Augmentation

37yo woman, 5’2"tall, weight 130 pounds who wished to have the breast size and fullness she did during her pregnancies. Prior to having children this woman wore a B-cup size, but following childbirth her breast size decreased to an A-cup. Five month post-operative photographs showing results of a bilateral dual plane-one, sub-pectoral breast augmentation with McGhan style 68 smooth, round, moderate profile (300cc) implants, placed via an inframammary fold incision, and filled to 330cc on the right and 325cc on the left. Post-operative bra size is a full 36C-cup, exactly what she wanted.

PATIENT 8 - Revision Breast Augmentation

34 yo woman, 5’10” tall, 160 pounds. This woman had an uncomplicated bilateral breast augmentation, through an inframammary fold approach, with McGhan style 468 textured, contoured 380cc breast implants, filled to 420cc. She experienced excessive right breast implant descent about five months later. One year later her initial surgeon placed internal sutures to raise the right inframammary fold about 1 cm; she noted some improvement but still felt there was a lack of inferior support for the right implant. In addition, she remained unhappy with the persistent breast asymmetry - the breasts were different shapes and there was more fullness in the upper pole of the left breast. Her bra size was a 36D-cup following her augmentation and revision, but she wished her breasts were a bit larger. Two years after her initial augmentation she sought to address her persistent dissatisfaction.

Seven week post-operative photographs show the result after an inframammary approach for removal of both implants, subtotal capsulectomies with additional capsulotomies, reconstruction of the right inframammary fold with internal sutures, and bilateral sub-pectoral breast augmentation with McGhan style 68 smooth, round, moderate-profile (450cc) saline breast implants filled to 480cc. Her post-operative bra size is a fuller 36D-cup, and she is happy with her improved symmetry.

PATIENT 9 - Breast Augmentation (and lift)

42 yo woman, 5’2"tall, weight 100 pounds who was unhappy with the loss of breast fullness and progressive drooping which occurred following her pregnancies and then worsened following some weight loss several years later. Pre-operative breast size is a B-cup. Eleven week post-operative photographs show the results following a bilateral dual plane-one, sub-pectoral breast augmentation with Mentor Lumera textured, contoured, high profile (305cc) silicone gel breast implants and a breast lift. Post-operative bra size is a full C-cup.

PATIENT 74 - Revision Breast Augmentation

52 yo woman, 5’4"tall, weight 114 pounds who had subglandular breast augmentation with 270cc silicone gel-filled breast implants through a periareolar incision in December 1987. She was treated for left capsular contracture with an open capsulotomy in March 1988, and required evacuation of a hematoma the following day. She was later treated with a right closed capsulotomy in January 1990.

She presented to our practice 16 years later with concerns regarding breast asymmetry and mild breast discomfort. Bra size at that time was 34C-cup. Examination showed asymmetry of breast size and shape. The right breast was globular (grade IV capsular contracture) and laterally displaced with the nipple-areolar complex positioned too high on the breast mound. The left breast had loss of upper fullness and grade III capsular contracture. The right nipple was 1.5cm higher than the left nipple. The overlying soft tissue coverage was thin. Contraction of the pectoralis muscles enhanced the visibility of the subglandular placement and capsular contractures. MRI demonstrated bilateral implant rupture with a small amount of extracapsular silicone on the right.

Eight month post-operative photographs show the results following removal of both ruptured implants with complete capsulectomy, removal of right extracapsular silicone gel, elevation of the right inframammary fold, site change to the subpectoral position with closure of the subglandular spaces, augmentation with Mentor Lumera textured, contoured, high profile (345cc) silicone gel-filled breast implants, and a right crescent inferior periareolar mastopexy. Post-operative bra size is a 34C-cup.

PATIENT 77 - Breast Augmentation

35 yo woman, 5’4"tall, weight 110 pounds who was unhappy with the loss of breast fullness following the birth and breast feeding of her son. Pre-operative breast size was a 34A-cup. Three month post-operative photographs show the results following a bilateral dual plane-one, sub-pectoral breast augmentation with Mentor high profile, smooth, round 325cc silicone gel-filled breast implants placed via an inframammary fold incision. Post-operative bra size is a 34C-cup.

PATIENT 79 - Breast Augmentation

37 yo woman, 5’5"tall, weight 139 pounds who desired larger breasts. She has previously undergone a left breast biopsy which was benign. Pre-operative breast size was a 36A to B-cup. Six week post-operative photographs show the results following a bilateral dual plane-one, sub-pectoral breast augmentation with Mentor moderate profile plus, smooth, round 325cc silicone gel-filled breast implants placed via an inframammary fold incision. Post-operative bra size is a 36C-cup.
